Pain under the second (2nd) metatarsal is usually due to capsulitis, metatarsalgia. On X-ray the metatarsal is usually longer than average and the metatarsal head is enlarged. This causes extra weight on the bottom of the foot. Bare foot walking, sandals, dress shoes, heels will irritate the bone and capsule. For more information on foot pain click here.
